Terms & Conditions:

1. This package is open to all self-paying outpatients during office hours only.
2. Optional add-ons must be selected during the initial consultation with the dietitian.
3. For all optional add-ons, all sessions must be used within 3 months and apply to individual sessions only.
4. All purchases are non-refundable and cannot be redeemed for cash or other items or services.
5. Any follow-up investigations, treatments, or medications as a result of this program will be charged accordingly.
6. Appointments must be made in advance and are subject to availability.
